APPLICATIONS:

2012-2013 RENEWAL APPLICATION for current Phillips Scholars

The postmarked deadline is April 2, 2012. Renewing students are not required to be residents of a New England state.

The 2012-2013 NEW APPLICATION FOR CURRENT HIGH SCHOOL STUDENTS 

This application is for high school seniors graduating in June 2012, or for high school graduates who have not been enrolled at a college or university on a full-time basis.  The application deadline is May 1, 2012.

The 2012-2013 NEW APPLICATION FOR CURRENT COLLEGE STUDENTS 

This is the application to use if you have attended at least one semester of college.  The application should be postmarked no later than May 1, 2012.

FINANCIAL INFORMATION: PAGE 8 OF THE APPLICATION

If your financial information changes after you have submitted your application through an increase or decrease in awards from your college or additional outside scholarships, please redo Page 8 and mail, attach to an email, or fax it to us.

FORMS: 

CHANGE OF ATTENDANCE FORM

  For temporary or permanent change of schools, or study abroad. You may complete the Change of Attendance form in your browser, then print it and submit it.
FERPA RELEASE FORM 

A release form that may be needed in order for some colleges' financial aid offices to share information with the Phillips Scholarship.
HOME SCHOOL FORM

Students who were home schooled for a major part of their high school years should include this form with their New Application. Current college students who are applying for a new award do not need to use this form.
ACCEPTANCE AGREEMENT FORM

Recipients, new and renewal, receive an award packet in mid-June which includes an Acceptance Agreement, which must be signed and returned in order to receive the award offered. If the paper copy is unavailable to the student, recipients may download, complete, and either print and mail it to us or save and attach it to an email to staff@spscholars.org.  If the Agreement is not returned by the deadline listed on the form, students may forfeit their award.
