Change of Attendance Form For temporary or permanent change of schools, or study abroad. You may complete the Change of Attendance form in your browser, then print it and submit it.

FERPA Release Form   A release form that may be needed in order for some colleges' financial aid offices to share information with the Phillips Scholarship.

Home School Form Students who were home schooled for a major part of their high school years should include this form with their New Application. Current college students who are applying for a new award do not need to use this form.

Acceptance Agreement 2010 Recipients, new and renewal, receive an award packet in mid-June which includes an Acceptance Agreement, which must be signed and returned in order to receive the award offered. If the paper copy is lost, it may downloaded, completed, and either printed and mailed to us or saved and attached to an email to staff@spscholars.org.  If the Agreement is not returned by the deadline listed on the form, students may forfeit their award.
