Applications and Forms

Applications for the 2017-2018 academic year are now available. Please read below for more information and specific deadlines. 

 

FOR CURRENT COLLEGE STUDENTS (new applicants)

The 2017-2018 New Application for College Students This application is for new applicants who have completed at least one semester of college and have never received an award from the Phillips Scholarship. The deadline is 12 noon, May 1st, 2017. It is suggested that applicants begin the application as soon as possible so that recommenders have time to submit their information. As soon as an applicant enters a recommender’s information in the application and saves the application, an email is sent directly to the recommender with a link to a personalized recommendation form for the student. All recommendation materials must be submitted by May 3rd, 2017.

FOR CURRENT HIGH SCHOOL STUDENTS

The 2017-2018 New Application for High School Students. It is suggested that applicants begin the application as soon as possible so that recommenders have time to submit their information. As soon as an applicant enters a recommender’s information in the application and saves the application, an email is sent directly to the recommender with a link to a personalized recommendation form for the student. Recommender materials are due by April 5th for students applying for an Early Response, and May 3rd for students who submit their application by the regular deadline.

  • Early Response deadline of 12 noon, April 3rd, 2017. This deadline is for students who would like to have a decision about the Phillips Scholarship before May 1st. The first 300 applications received by the deadline will be considered. (Submitted applications beyond the first 300 will be considered with applications submitted by May 1st.)   • Regular deadline of 12 noon, May 1st, 2017. Applicants will be notified in late June about a decision on their application.

FOR RENEWAL STUDENTS

The 2017-2018 Renewal Application. This application is for college students who are currently receiving or have received an award from the Phillips Scholarship. Please email the office if you are unsure about whether or not to use this application. The deadline for submitting a renewal application was 12 noon, February 1st, 2017. All applicants will be notified once their applications are complete about their award for 2017-2018. Official notifications and the acceptance agreement to sign will be sent in mid-June.

Recommendation Forms

High School Guidance Counselor Form Guidance counselors should receive an email recommendation request from each applicant. The link will bring the guidance counselor directly to the personalized High School Guidance Counselor Form. The form can be completed and submitted, along with appropriate, uploaded items:

  1. High school transcript, including senior grades
  2. Letter of recommendation written for college admission
  3. High school profile, if available

These should be completed and uploaded by May 3rd, 2017. (Due April 5th if student is applying using the Early Response option.) If guidance counselors are having difficulty receiving the email recommendation request from an applicant, please call our office at 978-744-2111 or email us at staff@spscholars.org. We can supply the direct link to the applicant’s personalized recommendation form. If all else fails, click on High School Guidance Counselor Form above. College Academic Recommendation Form College recommenders (professors, teaching assistants, or academic advisors) should receive an email recommendation request from each applicant. The link will direct the college recommender to the personalized College Recommendation Form. The form can be completed and submitted, along with an optional letter of recommendation to be uploaded. All recommendation forms are due by May 3rd, 2017. If College Recommenders are having difficulty receiving the email recommendation request from an applicant, please call our office at 978-744-2111 or email us at staff@spscholars.org. We can supply a direct link to the applicant’s appropriate recommendation form. If all else fails, click on College Recommendation Form above. FERPA Release Form This release form may be needed in order for some financial aid offices to share information with The Stephen Phillips Scholarship.
